Case Management

Case Managers are a vital part of our organization. They are committed to the success of our youth, and work to build rapport, assess needs, locate services, link participants to the services, and monitor progress.

Our case managers:

*Maintain contact notes
*Conduct ongoing assessments
*Develop case plans and review the plan with the participant every six (6) months
*Link participants to needed resources
*Provide advocacy services
*Conduct home visit
*Provide follow-up retention employment services

Mental Health Referrals

We are aware that a healthy mind, body and spirit are essential for overall health and well-being. Many of our youth come to us from broken backgrounds and in some cases, require mental health services. We work with Mental Health agencies to meet the needs of our clients.

Medical Referrals

The Independent Living Skills Teen Health Center opened during Summer 2004. We work closely with the staff to assist youth in gathering information and accessing services. we are fortunate to be located just across the hall from the clinic.
